FL-150 is California's official Income and Expense Declaration. You use it to give the court and the other party your financial information so the court can make orders about support, attorney fees, and other costs. It is not the form where you ask for custody or visitation - that is generally FL-300. FL-150 may accompany an FL-300 packet when you request child or spousal support.
